Hospital taxes doctors for keeping patients healthy.
To care for patients in the hospital, docs have to be on staff and pay yearly dues. I was paying $300/year. Then I got a bill for $700. Why? I only had one patient in the hospital last year. Docs admitting more than 25 patients/year still paid $300. I got fined because my patients were too healthy.
Same thing with my friend in his residency program. He helped his patients quit smoking, lose weight, reverse their diabetes. His program director called him into his office. He thought he was getting an award. Instead he was reprimanded for not generating enough money for the hospital with cardiac caths and other expensive tests. He got punished for keeping patients healthy.

    Ann, I love this idea for doctors! As a breast cancer survivor, my doctors were (and still are) the most important professionals in my life. Doctors' work is invaluable, and your peace of mind is critical to performing your jobs well. As a public high school teacher in Texas, we often feel as though people who have never taught make most of our decisions for us, too. It does indeed take a toll on one's mental health, which in turn affects our physical health. Perhaps this ideology will spread into the education field. I hope it does!     That’s wrong…it’s the same kind of problem that medical professionals with mental health problems face. Some are even sent to drug abuse programs when that’s not the problem. I’m a specialist in brain injury and a survivor of brain injury as well. I have worked with physicians and nurses who have been sent to drug “rehab” programs for brain injury symptoms even though their state board has been informed they had a brain injury and they tested clean. They couldn’t afford and shouldn’t have to go through this type of program for drug rehabilitation because it won’t fix brain injury. It just doesn’t make sense if they’re honest brokers. That’s the problem here…anyway, even with this situation, it’s so absurd. Hopefully, reclassifying marijuana will help somewhat. Who knows. I feel badly for this poor physician…hopefully she’s able to fight for justice for herself and others…

      @@PamelaWible I have a company that is focused on helping medical professionals that survive TBI…and figuring to protect them…as well as trying to educate the public at large on TBI…I may have a few docs and nurses and lawyers with brain injury who would be willing to…we have a board meeting on Monday…I’ll bring this up…I don’t know specifically what they think but some aren’t able to practice anymore because of disability and the assumption they can’t…I’m going to talk to our CMO, and see if she would sign and our CNO and a few of our docs and our two lawyers would. All of our people have brain injury (it’s a requirement of our company) so they’ve all experienced discrimination. I’m pretty sure you’ll have some others who’d support…

    If you have history of admission to inpatient psychiatry unit for attempted suicide, can that block your way of becoming a physician or getting into medical school?

      @@shapley4980 having two physicians as parents and then retiring my license was like closing up the family business. I DO think it was worth it for me bc I needed to understand these physician mental health issues (that have impacted me, my parents, and ALL docs IMHO). So essentially I would not be able to be a suicidologist who helps doctors (& medical professionals) without having experienced the unique & painful experiences I've had. Make sense?

    It's because they all have agreed to not cure. They can only treat illness. But to cure means loss of money. To choose money over curing people is pretty pathetic. You can bet that they have insider information for their own health. Maybe that's why most of these doctors look so healthy, young and vibrant. Their fear of going into a hospital is just too much to bear. But if by chance they do survive. Their inner turmoil will eat their insides up. You may look great on the outside, but eventually your mind will break down.

    I'm in my fourth year now and have started realizing this path ain't for me. The amount of mental health complications I've accumulated throughout my medical school experience has caused so much harm. Should I wait until I become a doctor and then quit? Just so I have a degree?

    I would have loved to read the letter he left behind.I think it should be made public so the public knows exactly what doctors have to go through.If a suicide letter is about the medical system, I think he would love for as much people to know the truth about the monster that made him take his life, maybe by reading his letter with profanities it would drive home the point in the rawest form possible.

How many doctors do you remember from 100 years ago and what lives they saved? Did what they do mattered? Maybe...maybe not...don't know...sigh...everything all of us have done can be undone by one mad leader with a bunch of nuclear weapons or a stray comet...or a supervolcano eruption...or a massive gamma ray burst that we won't know is coming till it hits...🤔 sorry...sorry...

    Psychologists and nurses too...I am a burned out paramedic and former ED/Trauma nurse...oh and did I mention two traumatic brain injuries...I've faced the examination and accusations...substance abuse? What? I can't touch alcohol or drugs because I have brain injury...I'm now a brain injury specialist...this abuse of doctors and other medical professionals breaks my heart...

    They also do this with other medical professionals too...psychologists, dentists, and nurses with brain injuries after an accidents...it's horrible the way treat those of us who dedicate ourselves to helping others...we need help, not punishment...I'm a survivor and a brain injury specialist...and this is so uncaring...we are human first...we get sick too...yes, I understand the need to protect the public...but there must be a more compassionate way to do this...and more accurate way...and you're automatically a disruptive physician, nurse, psychologist...whatever if you refuse to accept their judgment...what a bunch of crap...I'm sorry...this isn't legitimate at all...

    As an ex-medical student I confirm everything he says. It is sad really because they (schools) lose a lot of potencial brilliant doctors who decided not to deal with that nonsense and take their brains and time elsewhere. I would also add that part of this mental abuse is the teaching scheme where they load massive amount of useless information just for the sake of memorizing it. Medicine and dentistry is actually pretty logical but teaching system makes it unnecesserily over-complicated.

Also allowing for CONFIDENTIAL & NON-PUNITIVE mental health help is of critical importance.       SOLUTIONS: Medical boards do undermine physician mental health by breaching physician confidentiality and privacy. Discrimination against qualified, competent applicants who report mental health conditions is a violation of the Americans with Disabilities Act. In their search for criminal behavior among physicians, medical boards must not become criminal in their own behavior. By breaking federal law and the AMA Code of Ethics, boards have weaponized mental health diagnoses against physicians. Recommendations for all state boards: 1) Remove mental health questions from medical licensing applications. Replace with current impairment questions such as: “Do you currently have a condition that impairs your ability to practice medicine safely?” Comply with federal law by following best practices of Grade A states. Move criminal/predatory behavior queries to the criminal section alongside felonies and DUIs. 2) Address impairment from hazardous working conditions. Rather than focus on individual victims, engage in high-yield activities that resolve hazardous conditions impairing physicians en masse. To truly protect patients, align with all other industries invested in public safety that have legislated (and enforced) maximum 16-hour shifts, 60-hour work weeks, with minimum 30-minute breaks every 8 hours. 3) Encourage nonpunitive 100% confidential mental health care. Physicians require safe, accessible mental health care to be well-adjusted human beings. Most physicians enter medicine as humanitarians with noble intentions. Help them be well. After all, how can physicians give patients the care they’ve never received?     From what you've told us about Jon, it sounded like he was a pessimistic. Perhaps that was a bigger part in what led to his suicide? Either way, it's unfortunate he chose to end his life

      Perfectionistic more than pessimistic. He also was suffering with a cancer diagnosis and surely felt that he would not be able to save all the children he was capable of saving given his physical and mental health issues (part 2 to come) . . .

      Yes, you are absolutely correct. Competition, ego, emotional wounds, vicarious trauma can all result in a lack of empathy for others. We are 3x more likely to die by suicide than our patients.

    I’ve been under the care of a psychologist off and on for 10 years. No way would I ever disclose this on any of the licensing or re-credentialing applications I routinely fill out. I see it as civil disobedience in the face of an unfair system.

      You are in great company on that decision. . . . “Taking medication for ADHD is analogous to wearing glasses for my nearsightedness,” reports a psychiatrist. “My state licensing question asks, ‘Do you have a condition that could impact your abilities?’ As long as I am wearing my glasses I can see. As long as I am taking my ADHD medicine I can keep fairly focused. What business is it of theirs? Normally I feel guilty for lying about the slightest thing. I was counseled behind closed doors by a faculty who knew of my struggles with ADHD to simply put “no” on the form and leave it at that. This proved to be good advice. I was raised to be extremely honest about everything. When it comes to completing these questions for licensing I believe I am being honest because I was instructed by my respected faculty member to look at the questions in this light: Are you impaired by your condition?     I get people need to cope with "what comes with the job", but have respect. The job may have numbed them or whatever, but have empathy to the ones that are not viewing things on a daily basis.

      Have empathy for people who work in it everyday, you can't imagine it so you can't judge it. What about 1st responders? Friends and family experience it once and move on. When they are done with one horrific scene they top it off with another right away. Empathy works 8n more than one direction. It's not about numb. It's because you aren't numb that you have to find a way to cope. Dark humor is less destructive

    what would you suggest for people who still want to become physicians? With the knowledge you share about the abuses physicians faces, I feel those who still want to pursue this path will be mentally stronger than those who don't have this knowledge, right?

      Most important is to have informed consent so you can choose to enter the field knowing risks, benefits, alternatives (many ways to be a healer on the planet). Once you know medicine is the right field for you then essential to have a mentor who can guide you. Eyes wide open. Sharing info here to empower with truth.

      I think the majority of us in medicine enter with noble intentions and with unhealed childhood trauma (the decision to go to med school to help others is often because we are hurting -- see more on "Did your wounded child choose your career?" www.idealmedicalcare.org/did-your-wounded-child-choose-your-career/) SO . . . with all the vicarious trauma past and present from a career that pretty much beats the crap out of our humanity, some people crack and can't take it anymore and start acting in less-than-ideal ways . . . it's complex. . . I find it best to dig deep into the WHY so we can prevent injuring those who have such humanitarian intentions when they start this health care journey . . .

      @@maryduhon9769 actually i pray i am because there are racists and psychopaths and sadistic people. there are people who actively aim down. then there are oppressed people who are less malicious but still easily insecure and abusive and gossiping - and i believe the admin and you may be addressing them, and that may have individual context differences in amount style but the first portion of evil people finding great hiding spots in medicine is what i was addressing. im not torturing anyone and while all good in me is from god to not self oppress, thats not same as oppress others and def not as make a sport of trying to murder patients and employees, which is the population of racists in medicine i have 100% met and know are in plenty!!!!!

    On point. And the stress, not only of dealing with death, but also with all of the traumas and responsibilities of dealing with illness, is also the reason for "locker room" humor and teasing. The general public does not understand the burden, nor the role that hunor can play in coping.

      And vilification of first responders in news media regarding Kobe Bryant case really misses some of the underlying mental health issues that drive some of our maladaptive behavior. No excuse for passing around photos of dead bodies. I guarantee they were not laughing because they thought the tragedy was funny. My take on the bar scene is that many people go to bars and speak to bartenders to process a stressful event. And passing photos amongst themselves at the awards banquet (or even on their personal phones--BAD idea) was probably an act of peer support amid the nightmare of the event. No excuses. Again, just contemplating the underlying emotional scene at play.

    I really hope that everything worked out for this guy. He really tugs at my heartstrings because I too am a doctor who has been disillusioned with medicine for over 16 years. I have continued to tough it out because I am in a third world country where we earn very little and so money is the issue that keeps many of us trapped. Its amazing how long one can persevere through something that doesn't fulfill you or bring you happiness when there are limited options, bills to pay and dependents to care for....

    I think this is a cautionary tale. Don't go into allopathic medicine if you aren't able to sacrifice your long term goals of practicing for the short term. The education is long and grueling for a reason. You had the rest of your career to learn about alternative medicine or do whatever you want and you won't be limited, can practice anywhere, and don't have any administrative hoops because you paid your dues. There are tons of physicians who don't leave early and do what they want to afterwards. Also, many physicians don't even have the option leaving residency because they can't get licensed until they complete it. You can have a limited license if you leave after a year but only if you went to us medical school. If you're a prospective MD or a resident and struggle with the same burnout symptoms as this person, there are other options that won't limit your opportunities later in life. Don't give up if you're in residency watching this. You can take a leave, talk to your program, get treatment for anxiety and/or depression, you don't have to be perfect in residency, it's there for you to make mistakes and learn, you can buffer the scarcity because it is only temporarily. There are many other options such as adjusting rotations, electives, taking easier ones, transferring to a different program, ect. To be an actual medical doctor you do need to learn how to balance the challenges of practicing medicine in this capitalistic system. This woman, although very intelligent, caring, compassionate, and who would have made an amazing MD, drastically narrowed her future opportunities and doesn't know what she doesn't know. Had she stayed in the system, she may have developed other skills to get through it and the skills she learned by leaving would have been learned afterwards or could have also been learned during the program with self-directed, interest based scholarly/clinical activities. All the things she wanted to learn were always there for her. I don't like this video because she casually makes it seem like leaving residency can be a good option for burnout. It is most definitely not the option unless you don't want to be a doctor anymore. To me, this woman may be a healer but she is not a doctor. The difference is in the training and if you bail early you don't get the knowledge from the volume of patient's you treat. Residency is not being a doctor but it is needed to actually become one. Don't give up if your burnt out, speak to others, and keep your head up. The system won't let you fail if you stay in the arena to fight. Like all difficult things it life, good things come from sacrifice, take it one day at a time, and sooner than you realize you will be done and can actually create the credible clinic in any state that you want and do whatever you want.

      It's residency that causes burnout in the first place. Let's start from there. It's a system designed to extract resident labor at a much lower price for hospitals. You can separate capitalism and burnout. The former leads to the latter.

    Very brave for sharing. I'm a practicing MD, so I've made it through. but this is definitely all the truth. My college classmate who went into medicine unfortunately lost her life through suicide during residency.

      I'm so sorry for your loss. I enjoyed collaborating to create this Q&A. I agree with Dr. Wible down below, none of this is meant to take away the amazing nobility of this profession and how massively rewarding this can be. Nor is this meant to point fingers at anyone or organization. This is simply to open the discussion, pave the pathway to a better environment for future generations. I strongly believe healthcare functions at its best when clinical professionals are able to work at the highest level they are properly credentialed for + when healthy, empathetic, encouraging work environments are being promoted + everybody's job (from the CEO to the janitor at the hospital) is being respected. Healthcare is a team effort, and we are all there to serve at the best interests of the patients. Right now this simply cannot be sustained in the long run imo-and all data and trends point that way.
